Our 801455 Right Window Self-Seal Envelopes with Tint are sold by the Box of
500. They are made to fit the CMS-1500 Insurance Claim Form. They are on 24#
bright white wove paper with a clear cello window. They have turn-up, turn-down
self-sealing flaps and an inside tint for security. Prices shown above include
imprinting them in Black Ink with your name and address information.

How to Order Imprinted Envelopes
If we have printed them for you before, we will have that information on
file. If this is a new order, you may call, fax, or e-mail us with the
information you wish printed on the envelope. For new imprint orders, we will
send you a PROOF by your choice of fax or e-mail. You will have the opportunity
to make changes or approve the Proof. After Proof approval, the Envelopes will
ship within 7 working days.
