Our 800215 Blank Laser Statements are sold by the carton of 2,500 forms.
Each carton contains 5 packs of 500 forms. The form measures 8-1/2" x 11"
and is lightly screened on the front in blue ink. It is designed for
printing in laser and ink jet printers. It has a horizontal perforation
3-1/2" from the top so that it may easily be detached and returned with payment.
The form has no printed text, except for a message at the top that states you
accept MasterCardŽ and VisaŽ payments, a note to correct the recipient's address
if necessary, and a note to return the top portion of the statement with
payment. On the reverse side above the perforation are fields for entering
any changed personal information and credit card numbers for payment.
