Our Two Part Tamper Resistant Prescription Forms meet Washington State's requirements.  Each pad  contains 50 forms.  The form measures 5-1/2" W x 4-1/4" H.  Our prescription forms contain a minimum of Six security features: 1) "Void" Pantograph appears in the copy if the form is photocopied. 2) A green Washington State Seal in thermochromic ink vanishes with heat to prove authenticity. 3) A reaction in the paper will reveal the words "VOID" if chemicals are used to alter the form. 4) Attempts to erase portions of the form result in the removal of ink - making the alterations evident. 5) Flourescent Fibers embedded in the form prove authenticity. 6) "Security Prescription" watermark on reverse proves authenticity.

If we have printed them for you before, we will have that information on file.  If this is a new order, you may call, fax, or e-mail us with the information you wish printed on the prescription form.  For new imprint orders, we will send you a PROOF by your choice of fax or e-mail. You will have the opportunity to make changes or approve the Proof.  After Proof approval, the forms will ship within 5 working days.
