About Our PMA-18228 Sign In Sheet:
Our PMA-18228 Sign In Sheets are sold by the pack of 250 sets. Sets
measure 8-1/2" W x 11-5/8" H. Each set is a 2 part form with part one having
removable adhesive strips in the patient sign-in area. When a patient
writes on the sign-in sheet, a copy is transferred to part 2 of the form.
The entire line that the patient fills in may be removed from the sheet, and
with its adhesive backing, affixed to the patient's chart or other control
document. When complete, the sign-in sheet's second part serves as a
record of all patient visits.
